Community Health Workers and Hispanic CSHCN Meeting 2013

Last year, our MCHB supported National Center on Ease of Use of Community Based Services prepared a report on Reducing Health Disparities for Hispanic Children with Special Health Care Needs.  We documented the serious gaps in health insurance coverage and access to a Medical Home for Latino families. In the report, a strong set of recommendations focused on improving community based liaison (navigator) activities that have shown great promise in improving the situation for Latino families. This year we have been working on identifying successful liaison programs around the country that can stand as models of the best practices in true community and family engagement and partnership in the care of CSHCN.

We held a meeting with support from the HSC foundation in Washington DC on November 12 and 13 titled, "HIGH EXPECTATIONS”.  Below are various materials from that meeting:
